Due to a prolonged problem with computers, we were unable to answer and deal with letters and emails sent to us. This has resulted in a backlog, which we hope to clear as soon as possible.
I can confirm that we hold a service record for Stanislaw PONISZ and I attached two documents, which explain what is required before we can release information from the service records.

I regret to say, however, that I cannot immediately give you the information, which you seek. All Ministry of Defence personnel records are held in confidence and so to protect this confidence and in view of the Data Protection Act, which has recently come into being, we do not disclose details from them unless we have the written authority of the person concerned - the subject of the record.
If the above named are deceased, the official next of kin must give this authority. However, we need to establish who, is the official next of kin and so I am enclosing one of our “Certificate of Kinship” forms, which deals with this point and which you should complete and return. Once this is established you will be required to provide the following documentation:
Death Certificate of ex-serviceman and
- Marriage Certificate (in the case of widows)
- Birth Certificate (children) + Death Certificate of mother if applicable or written consent if
information is to be released to a person other than the widow
- Any other documents, which prove kinship
Photocopies of documents are acceptable
Information is issued free of charge to the widows of ex-servicemen. All other interested parties are/ charged/APC/POL/G £30 for the service. Payment can be made by cheque, UK Postal Order or International Money Order. These should be made payable to:
“Ministry of Defence – Accounting Officer”
I would like to add that payment from outside of UK can be made by Bankers Cheque or International Money Order (arranged through a bank e.g. Bank Polska Kasa Opieki SA. The bank should issue so called “Czek Bankierski”) These should be payable again, to “Ministry of Defence – Accounting Officer”. We are not able to accept money order.
